What can Funding Cover?

Minor home modifications, ramps, grab bars, etc.
Assistive devices, assistance with household setup, supplies, etc.
Personal support services (temporary), peer/ caregiver supports, and life skills coaching

Who Will Help Me?

Based on age criteria, either the Area Agency on Aging or your local Center for Independent Living (CIL)

An Options Counselor and Transitions Coordinator will meet with you and your support team to complete a review and personal needs assessment. Counselors will discuss a variety of services to help meet your needs.
