Who is Eligible?

• Individuals age 55 and older
• Reside in a Nursing Home/ Hospital for 20 days or more
• Meet Triage Risk Assessment Score

• Assistive devices, assistance with household setup, supplies, etc.
  
• Personal support services (temporary), peer/ caregiver supports, and life skills coaching

An Options Counselor and Transitions Coordinator will meet with you and your support team to complete a review and personal needs assessment. Counselors will discuss a variety of services to help meet your needs.
